CMS-1500 Form

The CMS-1500 is a particular format of health insurance claim form. The use of CMS-1500s for insurance claims is now required by many states. The Rosemark System can use the information you already have entered for a given Client and auto-fill the information into the appropriate sections of the CMS-1500, saving you some time and paperwork.

CMS-1500 forms are created from the Invoice List. To view the CMS-1500 for an invoice, make sure the invoice you want is open, go to the File menu, and click View selected Invoice CMS-1500 Form.

You can print CMS-1500 forms the same way, by selecting an invoice and clicking Print selected Invoice CMS-1500 Form instead of the View option.
